If space is available, parents are asked to come to the Center to complete an application packet.

This includes:

Medical statement form

Emergency Information

Insurance information

Parents school or work schedule

Emergency Contacts

Health Information


If space is not immediately available, parents may be placed on a waiting list. Please contact the appropriate New Sunrise Daycare Center for more information.

Needed Applications:

All the forms you need to enroll your child at New Sunrise are right here. Download them, print them and complete them when you have the time. Florida Certification of Immunization & Medical Evaluation is required for all new students. Then bring them with you to New Sunrise.
